	I Have Health Insurance from MediCare, Medi-Cal, State-Only Family Planning or Other Government Sponsored Programs and Wish to Become an AHS Member.
	I Do Not Have Health Insurance and Wish to Become an AHS Member or I Want to Check My Eligibility for Government Sponsored Programs Such as MediCare or Medi-Cal.
	I and my children DO Not Have Health Insurance and wish to know if we would qualify for Healthy Families .
